What is a Deed Poll?

A Change of Name Deed, also called Deed of Change of Name or Deed Poll is a legal document that enables one to formally change his/her name. Because it is a legal instrument it requires to be signed and witnessed before it is effective.

What is its purpose?

A Change of Name Deed acts as evidence that a person has changed his/her legal name. Without such a deed or similar documentary evidence government departments, companies and organisations do not normally accept a change of name.

What information is required?

Need full previous name, full new name and address. Also required is the reason for change of name and supporting document, if any.
